Jennifer Pernesky, RN, MSN
Jennifer Pernesky, MSN, (but we all know her as "Jen") is a Registered Nurse and the Chronic Care Coordinator at East Liberty Family Health Care Center. She has been married to Andrew Pernesky (who also works for the Center) for four years. They have two children - Dekker (2) and Emily (8 months). Jen is being honored this year for her impeccable service to our chronically ill patients, and for her dedication to the mission of providing the highest quality whole-person healthcare to our chronically ill patients.

Jen received a Masters of Science degree in Nursing from Clarion University where she trained to become a Nurse Educator. She has a Bachelor of Science in Nursing from Slippery Rock University, and a Bachelor of Science in Marketing from Indiana University of PA. She also obtained an Associates degree in Applied Science in Nursing from Butler County Community College. Before coming to the Center, Jen worked for the University of Pittsburgh Medical Center (UPMC) Presbyterian Montefiore. She worked in the Liver Transplant Intensive Care Unit where she served as a member of the Retention and Recognition Committee. Prior to that, she worked at UPMC Shadyside in the emergency department.

The quality of care that the Center provides to its patients has increased in large part because Jen joined our team. She works alongside Dr. Eileen Boyle, Interim Executive Director / Associate Medical Director, to coordinate services to our diabetic patients to help them manage diabetes more effectively. Because of Jen's proactive approach to patient care, 105 more diabetic patients received specialized care in 2009 than in 2008, and there were 66 more who received specialized care in 2008 than in 2007. More patients are being treated for diabetes each year because they can identify Jen as the face of Chronic Care Management.

The Center delights in honoring Jen at this year's Joyful Noise event. She has made such tremendous contributions both professionally and personally. She supports the Center because she is moved by the compassion shown to patients, and also amongst the staff. And needless to say, she is most passionate about the Diabetes Program because she understands the complexity of trying to managing the disease. Jen has set a goal of raising $1,500 to help sustain the use of the PPC chronic care model to help the Center better serve its more than 500 diabetic patients.

Jen's favorite song is The Words I Would Say by the Sidewalk Prophets. The words of this song are a description of Jen's work among us: Be strong in the Lord and never give up hope. You're gonna do great things. I already know God's got His hand on You so don't live life in fear. Forgive and forget--but don't forget why you're here.
